1095-B: Health Coverage
1094-B: Transmittal of Health Coverage Information Return
Who Must File
Every person that provides minimum essential coverage
to an individual during a calendar year must file an
information return reporting the coverage. Filers will use
Form 1094-B (transmittal) to submit Forms 1095-B
(returns).
However, employers (including government
employers) subject to the employer shared responsibility
provisions sponsoring self-insured group health plans
generally will report information about the coverage in Part
III of Form 1095-C instead of on Form 1095-B. These filers
may use Form 1095-B instead of Form 1095-C to report
coverage of individuals who aren't full-time employees for
any month during the year. In general, employers with 50
or more full-time employees (including full-time equivalent
employees) during the prior calendar year are subject to
the employer shared responsibility provisions. See the

information about who must file Forms 1094-C and
1095-C. Small employers that aren't subject to the
employer shared responsibility provisions sponsoring
self-insured group health plans will use Forms 1094-B and
1095-B to report information about covered individuals.

Step 1: Start ez1095 Software

After you installed the ez1095 software, you will see a new shortcut created on desktop. You can double click this shortcut to start ez1095 software.

Step 2: Set up Company Information
- Start ez1095 software, click the top menu "Current Company" then the sub menu "Company Settings" to view company set up screen.
- Enter company information
- Click the
SAVE button to save the change.
Step 3: Add Tax Form 1095-B
- Click the top menu "Current Company" then the sub menu "Form 1095-B" to view 1095B screen.
- Click the bottom link "Add 1095 B" to add a new form, enter data and save it.

Step 4: Print Tax Form 1095-B
You can print optional shipping address to mail forms with #10 double windows envelopes easily.
Step 5: Print Tax Form 1094-B
click the top menu "Current Company" then the sub menu "Form 1094-B" to view 1095B screen.
