1. Prepare Production Data
If your company data is already set up correctly, feel free to skip to the next page.
Add a New Account
- Start ez1095.
- Add a new account by clicking the top menu: Accounts → Add Company to add a new company account.
Enter Required Information
- Company Information: Fill out all necessary company details.
- 1095 Form Information: Enter or import the required 1095 form data.
- 1094 Form Information: Complete the necessary details for Form 1094.

Add Tax Form 1095-C
With ez1095 software, you can:
- Enter the form data manually by following the steps below.
- Roll forward previous year's ez1095 data.
- Import 1095-C data from spreadsheets.
- Import ACA form data from XML efile documents.
Steps to Add a 1095-C Form
- Click the top menu "Current Company", then select the submenu "Form 1095-C" to view the 1095-C screen.
- Click the bottom link "Add 1095-C" to add a new form, enter the data, and save it.
